We’re registered and accredited.
Support At Home (SAH)
We are registered and accredited to provide all Government-supported types of services for participants with Support at Home (Classifications 1-8) funding.
These include the Everyday, Independence, Clinical Support, Assistive Technology and Home Modifications (AT-HM), Restorative Care, and End-of-Life pathway services.

We begin by getting to know you: your goals, your routine, and the support you need to thrive at home.
Together, we’ll identify what matters most to you, what services will help, and what should happen during each visit to ensure your care feels personal, purposeful, and empowering.

Plan Your Care Budget
We work with you to map out the costs, explore funding options, and refine the plan until the care fits sustainably within your budget.
We understand that situations can change very quickly, and we are committed to being flexible.

Start Your Support
With the interim care plan and budget in place, we can start your services.
If you have a Home Care Package, we’ll guide you through signing your Home Care Agreement.
If you have a DVA referral, we will help you activate this referral to our service.

Assessment and Care Plan
Our care managers will work with you to complete all required documentation, and will perform further assessments to ensure your care is suitable.
Depending on how urgently services are required, we can do these assessments after starting services.

Ongoing Fine-Tuning
After you have started services, we work closely with you and our staff to ensure you are getting the services you need.
Typically we will make several schedule adjustments and modifications to the care plan over the first few weeks to ensure you are happy with the services you receive.
